Dyslexia United Grant Application
Dyslexia United helps families in need of financial assistance for their K-12 Minnesota students who have been diagnosed with dyslexia and require Orton-Gillingham based tutoring services to help the student reach their full potential.
Dyslexia United has monies to offer as grants to facilitate that assistance. The amount and number of grants awarded will be based on the combination of information provided in your application, and the amount of available funds Dyslexia United has to offer at the time your application is received. Applicants do not need to be Dyslexia United volunteers in order to apply for or receive grants.
To apply for a grant, please fill out the information below in its entirety and return it with all necessary supporting documentation to Dyslexia United. A committee regularly reviews all applications received and makes recommendations to the Board of Directors for final approval or denial. Incomplete applications will be returned.
All information provided on this form and necessary supporting documentation will be treated as strictly confidential and used only to determine eligibility.

Along with the above application, please provide the following:
1. Proof of Dyslexia: Proof of Dyslexia in the form of a letter from a qualified professional.
2. Verification that the grant is intended for Orton Gillingham based tutoring.
